Can acne be treated without antibiotics or isotretinoin (Roaccutane)?

Often, but it depends on severity, scarring risk and previous treatment. Mild to moderate acne may respond to an individualised combination of topical retinoids, benzoyl peroxide, azelaic acid or other evidence-based options, supported by suitable skincare. Hormonal therapy may help selected women. Oral antibiotics are sometimes appropriate for moderate to severe inflammatory acne and should be time-limited and combined with topical treatment. Isotretinoin can be the most effective option for severe, scarring or treatment-resistant acne and should not be dismissed when clinically indicated.

How do lasers and microneedling help acne?

Certain light- and laser-based devices may be used as adjuncts to reduce inflammation, target selected acne-related processes or improve post-acne redness, pigmentation and scars. Results vary by device, acne type and skin tone, and procedures do not replace an appropriate medical regimen for everyone. Microneedling mainly stimulates collagen to improve selected atrophic acne scars after active inflammatory acne is controlled; it is generally not performed directly through inflamed cysts or pustules. A clinician should assess pigmentation and scarring risks before treatment.

Does diet affect acne?

Diet can influence acne in some people, but it is rarely the only cause. Evidence suggests that a high-glycaemic dietary pattern may worsen acne for some patients, and dairy—particularly some milk intake—has been associated with acne in observational studies. This does not mean everyone needs to avoid dairy, gluten or entire food groups. Gluten restriction is not a routine acne treatment unless there is a diagnosed condition or clear individual link. A balanced plan should avoid unnecessary deficiencies and complement, not replace, medical care.

What if my acne is hormonal?

Hormonal influence is common in persistent adult acne, especially breakouts around the jawline or those that fluctuate with the menstrual cycle. Irregular periods, excess facial hair, scalp hair thinning or fertility concerns may suggest an associated condition such as polycystic ovary syndrome and justify further assessment. Hormone blood tests are not necessary for every patient. Treatment may combine topical therapy with selected hormonal medicines or management of an underlying condition, taking pregnancy plans and individual contraindications into account.

Is acne scarring permanent?

True depressed or raised acne scars usually do not disappear completely without treatment, although their appearance can often be improved. Flat red or brown marks after acne are post-inflammatory colour changes rather than structural scars and may gradually fade. The treatment depends on whether scars are ice-pick, rolling, boxcar or raised and may involve microneedling, subcision, peels, lasers or combination therapy. Active acne should usually be controlled first to reduce the risk of new scars. No responsible treatment can guarantee complete scar removal.

How long does acne treatment take to work?

Acne improves gradually. Many topical regimens need approximately six to eight weeks before a meaningful change is visible, and a first treatment course is commonly reviewed at around twelve weeks. Deeper inflammatory acne and post-acne marks can take longer. An early flare or irritation does not always mean treatment has failed, but severe worsening should be reviewed. Consistent use is important, and maintenance therapy may be recommended after control is achieved because acne can recur.

What is the best treatment for adult acne in Dubai?

There is no single best treatment for every adult. The plan should reflect whether acne is comedonal, inflammatory, nodular or hormonally influenced; its location and severity; scarring or pigmentation risk; previous treatments; skin sensitivity; medicines; and pregnancy plans. Dubai’s high ultraviolet exposure also makes non-comedogenic broad-spectrum sunscreen and pigmentation prevention important. Treatment may combine prescribed skincare, topical or oral medication and carefully selected procedures rather than relying on one facial, supplement or device.

Can acne treatment remove dark spots and post-acne marks?

Post-inflammatory hyperpigmentation can improve, but it often fades more slowly than the acne itself, particularly in deeper skin tones. The priorities are preventing new breakouts, avoiding picking or squeezing, using non-comedogenic broad-spectrum SPF 30 or higher, and selecting ingredients or procedures suited to the patient’s skin. Azelaic acid and certain retinoids can help selected patients, while peels, lasers or microneedling require careful assessment. Dark spots are different from indented scars and need a different treatment plan.

Is acne treatment safe during pregnancy or while trying to conceive?

Some acne treatments are unsafe in pregnancy. Oral isotretinoin must not be used during pregnancy, and spironolactone and topical retinoids are also generally avoided. Certain oral antibiotics are unsuitable at particular stages. Options such as azelaic acid and limited benzoyl peroxide may be considered for some patients, but the plan should be approved by the treating dermatologist and obstetric clinician. Tell the clinic if you are pregnant, breastfeeding or trying to conceive before starting medicines, supplements, peels, lasers or other procedures.
